The following documents are to be submitted when applying for the visa,

  1. Original Passport – Validity not less than 06 months
  2. Original medical certificate issued from a medical centre approved by the Gulf Health Council Expatriate Health Check-up program – gcchmc.org
  3. PCR certificate confirming that the passengers are free from the Coronavirus (Covid-19)
  4. Police clearance – Certified by the Ministry of Foreign Affairs
  5. For Driving Visa holder’s
    *“Driving information Extract” issued by the Department of Motor Traffic (Exempted for original Kuwait Driving License holders)
    *For reference bring Original Driving License
  6. For Family Visa holder’s
    *Medical Report – Exempted for ages below 12 years
    *Police Clearance – Exempted for ages below 18 years
  7. One way Air Ticket – Issued by Sri Lankan or Kuwait Airways (Printed on the relevant letter heads)
  8. Bank receipt obtained from Commercial Bank paid for Visa/Document fee

INSTRUCTIONS

Ministry of Health, State of Kuwait is accepting the Vaccination Documents issued abroad to be registered and validated by Public Health Department, so that it will enable all those vaccinated abroad (with Covid 19 Vaccines approved by Ministry of Health, Kuwait) upon approval, Register in Immune -مناعة APP and enjoy the same benefits /privileges as those who are vaccinated within the state of Kuwait.

Steps to Register for Citizens/Residents:

  1. Click the link or copy the link onto a browser and open for registration: https://vaxcert.moh.gov.kw/SPCMS/PH/CVD_19_Vaccine_External_Registration.aspx
  2. Select Residence Type (Select -> Citizens/Residences)
  3. Enter the Civil id Issued by Kuwait for Citizens and Residents, and an active email address which may be confirmed by re-entering in “Confirm email” Field.
  4. Submit the Send Verification code. You will now receive an One-time Verification code via Email to the Email address entered.
  5. Please enter this Verification code in the code Verification page.
  6. Upon validation you will be allowed access to registration page where you will fill the demographic details, Mobile Number.
  7. Fill in the field (name in English) as it is in the passport.
  8. Please select the vaccination dose to be recorded (First, Second, First and Second).
  9. Choose (first dose) if you have a vaccination certificate from outside Kuwait that contains data for the first dose only.
  10. Choose (the second dose) if you have a vaccination certificate from outside Kuwait that contains data for the second dose only.
  11. Choose (first and second doses) if you have a vaccination certificate from outside Kuwait with data for the first and second doses.
  12. Enter dose date.
  13. Choose the type of vaccination from the drop-down list.
  14. Choose a place or country from the dropdown list.
  15. Enter the batch number – mentioned on the vaccination certificate (optional).
  16. Upload the vaccine document issued by the appropriate authority of the Country from which you got Vaccinated. This document Should be a Pdf document of size less than 500 KB.
  17. The vaccination certificate or document must contain a QR Code.
  18. Agree to the declaration at the bottom of the page.

Once you are sure that all information entered is correct and have checked the declaration confirming that all information registered is correct Please submit for further processing. It will take three working days for the Public Health Department to verify and send the status of Approval of submitted Document and related information.

If approved, then download our IMMUNE App from Apple’s App store or Google Play store and Register to access vaccination details through Immune App.

Note: In the event that the certificate is accepted and approved for the first dose, you can register again for the second dose and follow the same previous steps, but using the same email used in the first registration. (When choosing a dose, choose only the second dose or two doses)

 

Steps to Register for GCC Nationals and Visitors:

  1. Click the link or copy the link onto a browser and open for registration: https://vaxcert.moh.gov.kw/SPCMS/PH/CVD_19_Vaccine_External_Registration.aspx
  2. Select Residence Type (Select -> Gcc) or (Select -> Visitors)
  3. Enter correct Passport number, confirm it and an active email address which may be confirmed by re-entering in “Confirm email” Field.
  4. Submit the Send Verification code. You will now receive an One-time Verification code via Email to the Email address entered.
  5. Please enter this Verification code in the code Verification page.
  6. Upon validation you will be allowed access to registration page where you will fill the demographic details, Full Name, Country dial code, Mobile Number and other details.
  7. Fill in the field (name in English) as it is in the passport.
  8. In case of registering as a Gulf citizen, please fill in the field (civil ID) for citizens of the Gulf Cooperation Council countries.
  9. If you are registered as a visitor
  10. Choose visit purpose from the drop-down list.
  11. Choose period of stay in Kuwait from the dropdown list.
  12. Enter expected arrival date.
  13. Enter National ID.
  14. Fill all vaccination details data.
  15. Enter dose date.
  16. Choose the type of vaccination from the drop-down list.
  17. Choose a place or country from the dropdown list.
  18. Enter the batch number – mentioned on the vaccination certificate (optional).
  19. Upload the vaccine document issued by the appropriate authority of the Country from which you got Vaccinated. This document Should be a Pdf document of size less than 500 KB.
  20. Upload Passport document, this document Should be a Pdf document of size less than 500 KB.
  21. In case if you registered as a visitor, Upload Passport + Visa visit, all in one document, this document Should be a Pdf document of size less than 800 KB.
  22. The vaccination certificate or document must contain a QR code.
  23. Agree to the declaration at the bottom of the page.

If approved, then download our IMMUNE App from Apple’s App store or Google Play store and register to access vaccination details through Immune App.

When logging in to the Immune App, enter the passport number and email used previously in the registration process, and you will receive a OTP code via email.

Benefits of Immune App:

For those who are Vaccinated with One dose, from the 14th day Immune app will display the status as Vaccinated (First dose Only) in “Green” thereby providing free movement to public locations like Theater, Malls etc. For those who are vaccinated with two doses, they will maintain their Green Status post Vaccination.

Those who are Vaccinated with One dose and have not passed 14 days, the status will be Vaccinated (First Dose Only) but in Red. Those who are not Vaccinated their Status will be “Not Vaccinated” in Red.

For those arriving from abroad, if their status is Green, then they will be sent for Home Quarantine. Those who are of Red status will be directed to Institutional Quarantine.
